If the dentist was pulling one of your teeth out then you may experience a significant loss of some kind- perhaps financial or possibly a painful and emotional loss.
Another perspective is that you need to remove a person, situation or issue from your life if it is having nothing but a negative effect on you. Do you need to put an end to a friendship or relationship if they are trying to make you do things you don’t want to? Do you need to leave your current job for something new and something you stand for? Or do you need to walk away from a situation that is causing you unnecessary grief because it doesn’t fall in line with your own morals?

If you are generally afraid of the dentist, then it’s possible you are fearful of something else during your waking hours that is filtering into your sleep state.
More simply, you may just have genuine concerns for the health of your mouth and teeth. Is your dentist check-up overdue? If so, it could be time to make an appointment. Or if your oral care regime could do with improvement- now might be the time to do something about it. Perhaps you need to address a little niggle where your dental health is concerned before it gets worse.
It’s possible you are dealing with someone at present who appears to be honourable and sincere on the surface but there is something about them that you distrust. Perhaps you have a tendency to see the negatives in people rather than searching for the good first. Should you trust your gut or are you being too sceptical about this person or people in general?
Another interpretation is that you are experiencing some temporary pain or anxiety right now- but you know deep down that it will be worth it in the end and the outcome will be much more pleasing than your situation now.
